Your first visit to Englewood Ear, Nose, & Throat PC, establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Please bring any recent radiology reports or test results that may be pertinent to your current problem.
Please note that we request cancellation of appointments 24 hours in advance. We charge a $50 fee for no shows and anyone unable to adhere to the above policy.
Patient Forms
Once your appointment is scheduled, you will receive an email directing you to our Patient Portal. This link will be live for 72 hours. Please click on the link and fill out the forms - they will automatically be uploaded to our computer system.
If you are unable to fill out the forms please arrive 20 minutes prior to your appointment time so you can fill out the forms on our kiosk in the office. If you are not familiar with using an i pad you must have someone with you to help you with this task. Please call our office with any questions @ 201-816-9800.
